Internal Community Engagement Survey

Change, Inc. is interested in your opinions as it regards additional efforts to build community within our environment, specifically around ways that ongoing events may be planned that are of interest to you clinically, administratively, and interpersonally-professionally.  Additionally, we are interested in your perspectives about what you believe is worth recognizing and rewarding within our environment, and how Change, Inc. can better engage in its efforts to do so.  Please complete each question below to those ends.
