Eligibility Terms:
--I understand that submitting this form means I become eligible for any hiring bonuses that are being offered as of the date of this form, provided the following terms are met:
Job Candidate Referral must remain employed for the specified length of time AND have receive a satisfactory performance review (i.e., no marginal or lower rankings in any category).
Job Candidate Referral must not have already contacted us -- i.e., Job Candidate Referrals who have already engaged our hiring process are not eligible to be referred for a hiring bonus.

Payout Terms:
--I understand that if others have submitted the same candidate's name using this form prior to the date of this submission, I will not be eligible for a referral bonus.   
--I understand that referral bonuses may be paid out over a series of checks rather than all at once.
